This is a #2 mirror (left side, y-axis) for a Universal Laser Systems machine.  It will fit platforms M, V, X, VLS, & PLS.  The ULS model number for this part is 620000100A.  This mirror was made by a local optics company that provides lenses and mirrors to laser manufacturers.
